Congestive heart failure, along with other cardiovascular diseases, is among the major causes of re-hospitalizations today. Our cardiac care nurses are specially trained to provide an advanced level of care in assessing, treating and monitoring cardiac patients after hospital discharge to help them stay healthy at home.

We work closely with your physician to create an individualized plan of care that focuses on early intervention of symptoms, monitoring patient response to medications and patient-family education. At the cardiac evaluation visit, our advanced level nurses conduct a head-to-toe assessment, complemented by comprehensive in-home testing. Equipped with portable EKGs, we take and interpret electrocardiogram readings, so at the first sign of irregularity, we can fax the results to the patient’s physician who can immediately adjust medications or order other treatments. 

Our TeleHealth remote monitoring program enables our nurses and our patients to keep track of their vital signs on a daily basis. Regular trend reports are also sent to the physician for ongoing tracking of the patient’s progress. As a result, patients don’t have to leave home to be tested at the doctor’s office.  Most important, our in-home testing and monitoring tools serve as vital early warning systems that have produced lifesaving results.
