Cardiac Disease Management

Reduce patient re-hospitalization with our care

We offer a cardiac disease management program to care for our many patients who have congestive heart failure or have been hospitalized for myocardial infarction (heart attack) or coronary artery bypass graft surgery. We excel in assessing, treating and monitoring cardiac patients after hospital discharge to help them stay healthy at home. 

Led by a registered nurse certified in critical care, our cardiac care team provides an advanced level of nursing assessment. We work closely with you to create an individualized plan of care for your patients. We focus on early intervention of symptoms, monitoring patient response to medications and patient/family education. At the cardiac evaluation visit, our advanced level nurses conduct a head-to-toe assessment, complemented by comprehensive in-home diagnostic evaluation, including arrhythmia and EKG monitoring, oxygen saturation rates, fluid status measurement and anti-coagulation testing.

  • Early intervention when abnormal symptoms occur
  • Improve patient compliance with medications and diet regimens
  • Enhance the disease management skills of patients and caregivers
  • Decrease re-hospitalization rates
  • Increase functional abilities
  • Improve quality of life

Services:

  • Cardiac evaluation nursing visit
  • TeleHealth remote monitoring 
  • In-home arrhythmia and EKG monitoring, oxygen saturation rates, fluid status measurement and anti-coagulation testing
  • Patient and family education regarding medications, disease process and prevention, symptom management, daily weight, reportable signs and symptoms
  • When necessary, physical and/or occupational therapy, social work and home health aide services to improve quality of life
  • Evaluation of outcomes based on improved endurance, comfort, reduction in re-hospitalizations
