VNSW offers a number of unique specialty programs, designed
to enhance quality of life – for patients, their caregivers and
families – plus independence for patients and the ability for them
to receive as much treatment as possible in the comfort and security
of their own homes.

TeleHealth
Monitoring – (914) 682-1480, Ext. 635
TeleHealth is a methodology that allows for remote monitoring of patients
in their homes and two-way communication to manage their care and self-care.
The TeleHealth device, used like a modem, also has ability to ask questions
of the patient and provide immediate feedback in the form of educational
coaching. VNSW integrates TeleHealth into the treatment of patients
with Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease
(COPD), Diabetes and other conditions.
Cardiac Disease
Management – (914)

Led by a Registered Nurse certified in Critical Care,
the Cardiac Team provides an advanced level of nursing assessment and
utilizes a multi-disciplinary plan of care. In-home EKG monitoring, oxygen
saturation rates, fluid status measurement and anti-coagulation testing
can be provided as needed in the home.
In-Home
Mental Health Care – (914) 682-1480, Ext. 648
Registered nurses with advanced psychiatric training provide therapeutic
services for individuals with mental health issues. Mental Health nurses
design a home care program utilizing a case management approach led by
a Psychiatric Nurse Specialist. The program serves adults, the elderly,
children and adolescents.
Pain
Management & Palliative Care – (914) 682-1480, Ext.

A multidisciplinary team – led by an Advanced Practice Nurse
certified in pain management and palliative care – that provides
home-based care to those suffering from acute and chronic pain and/or
other end of life physiological symptoms.
Advanced
Wound & Ostomy Care – (914) 682-1480, Ext. 653
A
team of registered nurses with specialized training treats the complex
and challenging disorders of the gastrointestinal, genitourinary and
integumentary systems that have resulted in open wounds and stomas, which
require advanced nursing management. Conditions treated include acute
and chronic draining wounds, fistulas, dermal ulcers, ostomies and issues
surrounding incontinence.
Joint
Replacement Rehabilitation – (914) 682-1480,

An all-inclusive rehabilitation approach for the treatment of
Total Knee and Total Hip Replacements, this program includes pre-surgical
interventions and post-operative treatment following patient discharge
from a hospital or sub-acute/rehabilitation facility.
Dysphagia
(Swallowing Disorders) Rehabilitation – (914) 682-1480, Ext.

A program treating difficulty or discomfort with chewing and swallowing,
plus comprehensive Speech Pathology Rehabilitation Services. Evaluation
and treatment are provided in the home, with emphasis placed on safety
strategies to assure adequate nutrition/hydration and reduce the risk
of aspiration
Tobacco Cessation – (914)

Assists the homebound patient in recognizing the dangers
of tobacco use and second hand smoke. Staff have been trained in strategies
for assisting patients who are interested in quitting. A Tobacco Cessation
Specialist is available for consultation and counseling for patients
and their families.
