Home > Specialty Programs > TeleHealth MonitoringTeleHealth MonitoringVNSW has been on the cutting edge of TeleHealth-remote patient monitoring since 2007. TeleHealth allows the patient to remain in their home while being monitored daily by an Registered Nurse. The TeleHealth Program began in conjunction with the VNSW’s Cardiac Disease Program, primarily monitoring patients diagnosed with Congestive Heart Failure. The TeleHealth Program has now expanded to many other diagnosis; including but not limited to Chronic Obstructive Pulmonary Disease (COPD), Hypertension/Hypotension, End Stage Cardiomyopathy, Anemia, Obesity/Failure to Thrive, and Diabetes. TeleHealth is a methodology that monitors patients remotely in the privacy of their home, with the Registered Nurse reviewing and analyzing data daily. Readings are monitored by the TeleHealth Registered Nurse from a computer, either at VNSW headquarters or any remote location. The TeleHealth Nurse calls the patients, welcomes them to the program, and provides information on how to do the readings, as well as support to overcome any anxiety that maybe experienced about using the TeleHeatth equipment. The Registered Nurse looks for trends in the data (i.e. early morning elevated B/P that normalizes after the medication is taken, or a gradual weight increase). The Registered Nurse reviews the trends with the patient to identify potential behaviors that may have contributed to the abnormal readings, while coaching to assist the patient to identify behaviors that have will have a positive effect on the over all well being of the patient. The trends are also reported to the Physician for further review/investigation and possibly for medication changes. This real data becomes actionable data. The Physician feels confident making changes because the data is actual data, not self reported data. Additionally, Weill Cornell Medical College’s Institute for Geriatric Psychiatry in White Plains, has partnered with VNSW to develop a depression program, using TeleHealth. Services
