As adults transition through different life stages, our goals and needs often change dramatically. This is especially true for older adults who may be facing changes in their health status, lifestyle and social relationships. However, many adults adapting to new circumstances may find it challenging to identify and navigate the services and support systems that can provide them and their families with a greater sense of well-being and peace of mind.
To help guide adults through life transitions, VNSW’s Communty Care Navigation Program (CCN) provides comprehensive life planning, care navigation and wellness services in the comfort of your home – in-person or virtually. Thousands of people have entrusted VNSW (Visiting Nurse Services in Westchester) with their personal care needs for over a century. Our CCN program builds on our tradition of providing the highest caliber of care to those who wish to age in place safely, and achieve the best possible quality of life. CCN services offer unsurpassed support by our expert Care Navigators who understand the needs of and resources available to older adults and individuals with special or complex needs.
A Customized Plan for You
Our Care Navigators will work closely with you and/or your family to create your individual life plan, with components that take into consideration your goals and wishes regarding:
- Physical and cognitive wellness, including health history, medications, exercise
- Social and emotional wellness
- Legal options, including health care proxy and advanced directives
- Occupational goals, including life activities, meaningful work, joy in accomplishment
- Living environment, e.g., safety modifications and technology to age in place
Your Personal Health Care Navigator
- Accompanies you to MD, ER and hospital visits, including after hours on-call service
- Supervises transitions between care levels, ensuring your safety and care continuity
- Ensures timeliness of start of care for agency, private duty and other services
Securing and Maximizing the Entitlements You Need and Deserve
- Explanation and completion of plan enrollment applications (e.g., Medicare, Medicaid)
- Coordination of referrals to partner agencies, as requested
- Identification of community resources, as needed
EMPLOYER SERVICES
VNSW @ CCN also provides Care Navigation Services for employers to help them assist employees grappling with life transitions that may impact productivity and career development.
