In the aftermath of a serious illness or injury, patients need a team of professionals to help them recover and improve their health at home. We know how stressful it can be for families or other caregivers to assemble that team, especially when needs arise quickly.

VNS Westchester is here to ease your concerns.

We’ll respond to your request for home health services and arrange care within 24 hours. We’ll answer all of your questions about our services, insurance coverage and much more.  We’ll send a skilled nurse to assess your loved one’s condition, pull together your care team and coordinate services, working closely with your physician.  As your needs change, your nurse will adjust your care plan accordingly, so you are always receiving the care you need when you need it.

Our basic coordinated services include:  

SKILLED NURSING AND CASE MANAGEMENT

A Registered Nurse, trained and experienced in home healthcare, evaluates patient needs; performs skilled nursing procedures (e.g., dressing changes, injections, etc.); and educates patients, family members and caregivers about the patient’s condition and treatment, self-care techniques and prevention strategies. The nurse coordinates patient care and services with your physician and other VNS Westchester healthcare team members.

PHYSICAL, OCCUPATIONAL AND SPEECH THERAPY

Based on your plan of care, a licensed therapist provides skilled rehabilitative services designed to help  patients achieve optimal health and independence. Physical therapy involves evaluation and treatment of mobility issues including post-surgical care, exacerbation of chronic conditions, debility/weakness  (e.g., via exercise); Occupational therapy helps patients return to everyday activities through training and use of adaptive equipment; Speech therapy addresses problems involving speech, language and swallowing.

MEDICAL SOCIAL WORK

Medical social workers assist with social and emotional concerns related to illness. They provide short-term counseling services, make referrals to and coordinate with community resources, and assist with living arrangements, finances and long-range planning.

HOME HEALTH AIDES (In Conjunction with Skilled Care)

Home health aide services may be designated as part of a patient’s plan of care in conjunction with skilled nursing and therapy services provided by VNS Westchester. The specific home health aide services provided, as well as the length and frequency of services, are based on the patient’s personal care needs. Typically, Medicare may cover such personal care services as: bathing/showering; dressing; feeding; changing position in bed; help getting in/out of bed; and exercises as instructed by the nurse or therapist.

MEDICAL EQUIPMENT AND SUPPLIES

We provide essential medical supplies needed to carry out home care visits (e.g., catheters, dressings, etc.)  in accordance with Medicare regulations or other payer considerations. We can arrange delivery of durable medical equipment, such as a walker, wheelchair, hospital bed and oxygen, as ordered by the physician and based on your insurance coverage.

IN-HOME ANTI-COAGULATION TESTING

Our skilled nurses can conduct many medical tests in conjunction with skilled services, including anti-coagulation testing, in the home, so you are not required to leave the home to use an outside testing service.

EVALUATION FOR NURSING HOME PLACEMENT

Our PRI (Patient Review Instrument) and screen certified nurses will complete these forms in your home to assist in transition from home to nursing home.

LANGUAGE TRANSLATION BY CERTIFIED MEDICAL INTERPRETERS

We provide support in more than 150 languages and dialects. We also can provide patients with bicultural nurses by request.
